Medical Records Clerk
Prepares correspondence accurately and timely (required time frame - 14 business days in accordance to VA State Guidelines)


Maintains correspondence log reflecting all correspondence received and/or mailed in accordance to 45 CFR HIPAA Laws.


Distributes all correspondence requested by referral sources in accordance to signed contracts/agreements.


Insures faxed information is transmitted to a secure fax location in accordance to policy.


Processes FMLA/STD forms in an accurate and timely manner.


Codes and indexes diagnosis in an accurate and timely manner.


Maintains transcription log for dictated documents.


Maintains "Dictation to be Transcribed" log.


Audits medical record for completeness (including new admission audit, final chart audit in a timely manner - 24 hours of admission /72 hours on holidays and weekends).


Organizes, collates, files, indexes medical records for discharged patients according to guidelines.


Maintains patient information systems accurately.


Proofreads transcribed documents for completeness and accuracy. Makes corrections as appropriate and distributes and


files accordingly.


Coordinates with other staff to maintain patient confidentiality in accordance with CFR 42 & 45 laws & VA State Guidelines.


Prioritizes and follows through with critical correspondence items.


Assist other departments with unusual problems encountered with medical records.


Prioritizes and transcribes dictated materials completed by professional staff promptly after receipt.


Collates and assembles discharged patient records according to department guidelines within 24 hours of discharge or 72 hours of discharge on holidays or weekends.


Maintains and insures accuracy of daily census information on admissions/discharges and relays any changes to appropriate departments.


Monitors release of information forms for proper completion to help insure patient's rights are protected.


Advises Medical Records Manager or Administrator of any unusual or difficult problems encountered.
